This policy explains how Rebuild Therapy Infusions Center handles information collected through this website. It does not replace the Notice of Privacy Practices you are given as a patient, which governs protected health information under HIPAA and is provided to you in clinic.

Clinical information you provide during an in-clinic intake is held in your patient record and governed by HIPAA and Florida law, not by this policy.
How we use it
To respond to your enquiry, schedule and confirm appointments, and provide the care you request. To improve the website. To meet legal, regulatory and record-keeping obligations.
We do not sell your personal information. We do not share it with third parties for their own marketing.

Website enquiry data is retained only as long as needed for the purpose it was collected or as required by law. Patient records are retained under the schedule Florida law sets for medical records.
We use reasonable administrative and technical safeguards to protect information. No method of transmission over the internet is completely secure, so please do not send detailed medical information through a web form or by email. Call the clinic instead.
Your choices
You may ask us to correct or delete the contact information you have given us through this site, and you may opt out of marketing communications at any time. Requests about your medical record are handled under the separate HIPAA process described in your Notice of Privacy Practices.
